1) Your Personal Plan for Care will challenge you to anticipate the care you would like to receive in your twilight years or in the event of a life changing injury or Illness. Through the course readings and modules reflect upon your preparation. Begin writing notes on issues you will encounter, for example: a) the place of care, b) the type of care you feel fits you best, c) challenges you will face in the terms of finance; d) care options ;e) regulations; f) family caregiver availability; and other needs. Example: My father died after a seven year experience with dementia, during which he was cared for at home. Our mother and his ten children, ten spouses, and 30 grandchildren all found some way to participate in his family centered care. Care at home impacted both Dad and Mom’s quality of life. Without one hospitalization over those seven years,
Dad’s expenses were minimal. With his dementia, our planning was to avoid hospitalization, avoid tube feedings, provide comfort care. As a Medicare beneficiary Dad had hospice for the final ten months of his life. His care and death at home is now part of our family story. The final paper is to be six pages in length not including the title and reference page, in APA format, to include at least ten scholarly references, and should be grammatically correct, with proper spelling and punctuation. As in the draft, the sections to be covered include: a) the place of care, b) the type of care you feel fits you best, c) challenges you will face in the terms of finance; d) care options; e) regulations; f) family caregiver availability; and other needs you find important and wish to add.
